The following information will help you know what to expect.

Your First Visit

Your first visit will last approximately one hour and includes a comprehensive health history interview, several in-office tests, a review of your recent lab tests and and review of medications and supplements, and a brief physical exam if there is time. Together we will outline a health program to get you started. We encourage you to bring your spouse or significant partner.

If you are involved in one of our programs, Genetics, Joint regeneration, Stem Cell, Cellular rejuvenation, we launch into the first step. Be prepared to talk about your health goals for the next 6 months, year, 5 years.

Second Appointments will last 30 -45 minutes and we will discuss and explore the labs and what your results mean in relationship to your symptoms and health goals.

To maximize your time with our doctors, please contact us for required forms to fill out. These forms will take some time to complete, as several of them require a few days of logging information. Please review them immediately so you can plan your time accordingly. Your paperwork must be completed prior to your appointment time.

The programs and packages are not a short-term fix, but an introduction to an achievable , long-term lifestyle that lasts. These programs are customized healing programs of 4,6,8 months in duration and designed individually, based on patient education. At the core of each patient’s program is a support system to achieve the lifestyle changes necessary to heal and an education designed to give you the tools and confidence you need to take charge of your health.

OFFICE POLICIES

Cancellations require a minimum of 24 hours notice. You will be billed for your appointment without 24 hours notice.

Payment in full is required at the time of your visit. We accept Visa, Mastercard, American Express, cash, checks and We Trade (for services only).

We do not bill insurance carriers directly; however, we are happy to provide you with the necessary paperwork for you to submit to your insurance carrier.

Please also bring:

  • Any laboratory reports from tests conducted in the last two years. (If you do not have your own copies, the Release of Records form will allow you to authorize other physicians to send copies of those reports to us.)
  • Any vitamins, supplements, and medications you are currently taking.
  • Patient Policy Form
  • Comprehensive Intake
  • Diet Journal and Body Temperature Log
  • Nutritional Informed Consent
  • Notice of Privacy Practices (HIPPA)
  • Release of Records

Office Hours are Tuesday-Friday from 9am – 4pm. The dispensary is open during these hours as well. Please let us know your needs in advance, so we can prepare your items for you. Please allow 24 hours notice on items made in-house.
